Evolution of Magnesium Sulphate for Eclampsia

چکیده
Eclampsia is said to occur when a woman with preeclampsia experiences generalised tonic-clonic seizures during pregnancy or shortly after delivery [1]. Eclampsia complicates between 1 to 2% of all cases of severe preeclampsia [2]. Preeclampsia is a multi-systemic disorder, characterised by hypertension and proteinuria occurring after the 20th week of pregnancy in a woman who has been previously normotensive and non-proteinuric. It is classified as one of the hypertensive disorders of pregnancy [1,2]. Eclampsia was first associated with albuminuria in 1839, before hypertension in 1897 [3]. These discoveries, coupled with the introduction of antenatal care in the first decade of the 20th century, led to better understanding and earlier diagnosis of the precursor condition known as preeclampsia. In spite of this, mortality from eclampsia still remained high in the 20th century, necessitating active search for treatment [3]. The low and middle income countries suffer a disproportionately higher morbidity and mortality from preeclampsia and eclampsia. The incidence of eclamptic convulsions reported in developed countries is 1/2000 while in developing countries; the incidence varies from 1/500 to 1/50 deliveries [4]. Most (99%) of the 63,000 women worldwide that die every year from preeclampsia and eclampsia are in low-income countries [5]. The reported case fatality rates from eclampsia range from 1.8% in some centres in high-income countries, to as high as 15% in Nigeria and Bangladesh [6,7]. This condition accounts for 25% of maternal deaths in Latin America and 10% of maternal deaths in Africa and Asia [8].

Determinants of magnesium sulphate use in women hospitalized at <29 weeks with severe or non-severe pre-eclampsia
OBJECTIVE Magnesium sulphate is recommended by international guidelines to prevent eclampsia among women with pre-eclampsia, especially when it is severe, but fewer than 70% of such women receive magnesium sulphate. We aimed to identify variables that prompt Canadian physicians to administer magnesium sulphate to women with pre-eclampsia. METHODS Data were used from the Canadian Perinatal Net...
متن کاملLow Dose Magnesium Sulphate Regime for Eclampsia
13 LOW DOSE MAGNESIUM SULPHATE REGIME FOR ECLAMPSIA Bangal V., Kwatra A., Raghav S., Jadhav S. Abstract Preeclampsia is one of the commonest medical complications seen during pregnancy. It contributes significantly to maternal and perinatal morbidity and mortality. Dr.J.A.Pritchard in 1955, introduced magnesium sulphate for control of convulsions in eclampsia and is used worldwide. Considering ...
متن کاملA Study to Compare the Efficacy of Low Dose Magnesium Sulphate (dhaka) Regime with Pritchard Regime in Eclampsia
Background: Eclampsia is a hypertensive disorder related to pregnancy, in which there is occurrence of one or more generalized convulsions and or coma, in the absence of other neurologic conditions. It is a common obstetric emergency. Eclampsia is associated with elevated maternal and fetal morbidity and mortality. Objectives: To compare the efficacy of low dose magnesium sulphate (Dhaka) regim...
متن کاملMaternal outcomes of magnesium sulphate and diazepam use in women with severe pre-eclampsia and eclampsia in Ethiopia
BACKGROUND Preferred anticonvulsant used to treat and prevent fits in eclampsia currently is magnesium sulphate. Clinical monitoring of tendon reflexes, respiration rate and measuring hourly urine output should be done to ensures safe administration of magnesium sulphate. OBJECTIVE This study was conducted to evaluate maternal outcomes of magnesium sulphate and diazepam use in the management ...
متن کاملA shortened versus standard matched postpartum magnesium sulphate regimen in the treatment of eclampsia: a randomised controlled trial.
Magnesium sulphate is currently the most ideal drug for the treatment of eclampsia but its use in Nigeria is still limited due its cost and clinicians inexperience with the drug. The purpose of this study was to determine whether a shortened postpartum course of magnesium sulphate is as effective as the standard Pritchard regimen in controlling fits in eclampsia Between January and June 2011, 9...
متن کاملWhich anticonvulsant for women with eclampsia? Evidence from the Collaborative Eclampsia Trial.
Eclampsia, the occurrence of a seizure in association with pre-eclampsia, remains an important cause of maternal mortality. Although it is standard practice to use an anticonvulsant for management of eclampsia, the choice of agent is controversial and there has been little properly controlled evidence to support any of the options. 1687 women with eclampsia were recruited into an international ...
